Yes.

Your medical record has been created electronically and stored digitally. We can provide you with PDF copies of your medical record on a USB flash drive at no charge.

If you want paper copies of your medical record, your digital files will be downloaded and printed by a copying service and mailed from a mailing center. You will be billed for the materials and labor charged by the copying service and the cost of mailing. The cost will depend on the number of files downloaded, printed, and mailed. Payment must be received before the medical records are sent.

Are some parts of my medical record more important than others?

Yes.

Your most recent progress notes are the most important part of your medical record. Progress notes contain a list of your current medications, drug allergies and intolerances, a summary of your past medical history with pertinent supporting data and information, your surgical history, family history, social history, physical findings, testing results, and medical assessments. Copies of your recent progress notes, electrocardiograms (EKGs), and other pertinent supporting data, will provide you and other healthcare providers information needed to assist in your care.

Can I select which portions of my medical record I want?

Yes.

When you request a copy of your medical records, you can designate which sections you would like. Generally, copies of recent progress notes and pertinent supporting data are the best and most useful option, but you are entitled to a complete copy of all the records if you so desire. If you opt for a complete copy on paper, the cost could be significant. We can provide a complete copy of your medical records on a USB drive at no charge.

How do I request a copy of my medical record?

The Health Insurance Portability and Accountability Act of 1996 (HIPAA) is United States legislation that provides data privacy and security provisions for safeguarding medical information. Under HIPAA regulations, we will require your signed medical record release authorization to provide you with a copy of your medical record. You can then share that information with healthcare professionals or others that you deem appropriate.
